? Location: Remote, but candidates should live near Shreveport, LA

? Full Time ? No Weekends
? Negotiable Pay/ Weekly Compensation
? Duration/Schedule: 3 months with likely extensions
? Patient Volume: Complete 3+ patient visits daily
Responsibilities
? Maintain quality of care standards
? Assist patients with enrolling in educational programs
? Knowledge of disease diagnosis and prevention
? Assess and evaluate patients' health status
? Implement and follow up on care plans
? Document patient information comprehensively and timely
? Counsel and educate patients and families about health benefits and programs
Requirements
Experience: 2 years of direct patient care relating to polychronic conditions Polychronic conditions, also known as multiple chronic conditions (MCCs), refer to the presence of three or more chronic diseases in a single patient. These conditions can include but are not limited to diabetes, cardiovascular disease, cancer, and more.
